General Consent including Privacy
We respect your rights to privacy and take our privacy obligations seriously. Our full Privacy Policy can be obtained here, our clinic reception or by calling 1300 252 698.
We require your consent to collect personal information and health information about you. Please read this information carefully.
BJC Health collects information from you for the primary purpose of providing you care for the medical problem about which you consulted us. We require you to provide us with your personal and health information including your full medical history in order to service your needs. We will also use the information you provide in the following ways:
- Management of our practice, such as the conducting of audits and undertaking accreditation processes, managing billings and training staff.
- Effectively communicate with third parties, including Medicare Australia, private health insurers, government departments and other practitioners involved in your healthcare.
- Use de-identified clinical and demographic data for the purposes of clinical audits, research and quality assurance.
- This information will only be passed on where appropriate for the care of your medical/health issue about which you consulted us, or where legally required. Please let us know if you require further information.
- The practitioners in this practice will discuss your condition and options for treatment with you so that you are appropriately informed and can make decisions relating to treatment/management. You may choose to consent to, or refuse any form of treatment for any reason including religious or personal grounds. Once you have given consent, you may withdraw that consent at any time.
- During the examination, assessment, and treatment it may be necessary for your practitioner to make physical contact. Your practitioner will ask your permission before making physical contact with you in anyway.
- As part of quality assurance, BJC Health may engage a third party to conduct satisfaction surveys which will require BJC Health to provide your name and contact details. Please note that no other personal or health information will be disclosed.
- Your mobile number will be used to send you appointment reminders whilst your email address will be used to send you information about BJC Health, pre-payment links and information relevant to your care.
Financial consent
BJC Health does not offer account facilities - all treatments must be paid for at the time of or prior to the consultation. This excludes most patients who are funded through workers compensation/CTP/life insurers, DVA and NDIS. Where applicable, BJC Health will claim your Medicare benefit online when processing your payment, unless advised otherwise.
Should you need to reschedule your appointment, please inform us at least 24 hours prior to your appointment time. Broken appointments with insufficient or no notification may incur a fee with recurrent offenders asked not to return.
Please also note:
- $20 processing fee for rewriting of prescriptions, pathology/ radiology referrals, travel medication letters, and miscellaneous reports or letters.
- $30 administration fee for a compassionate supply of biologic medication
PATIENT DECLARATION: I have read and agreed with the information contained in this document and I understand why my information is collected and how it is used. All my answers and information provided by me are, to the best of my knowledge, correct. Should there be any changes in the future, I will notify the clinic of them as soon as possible. I acknowledge that I am not obliged to provide any information requested of me, but that failure to do so may compromise the quality of care provided to me.
In the case of insurance claims, I give my consent for all appropriate, non-confidential information to be released to relevant parties (employers, insurance companies, etc.). Should my claim not be accepted, I understand that I am personally responsible for payment of accounts for services received here.
